Spring physicals will be held on Saturday, May 21nd, 2011 from 8am until 11am in the gym. No one will be let in after 11am. Please have all paper work ready and properly completed and cash or check in the amount of $25.00 made out to SMCHS upon arrival. Physical form and emergency cards below. This physical will cover students for the 2011-2012 school year.
If you are unable to attend, South Coast Medical Group offers walk-in visits to get your physical clearance. Please take this form filled out along with the physical form.
 Physical Form - This annual physical clearance is required for all students participating athletics at SMCHS. Emergency Card - We require two (2) forms. Please detail phone numbers and emergency contacts. The athletic training staff and coaches use this information in cases of emergency when parent/guardian contact is required.DEADLINE: All students are required to submit a physical clearance and emergency card before participating in try-outs or start of practice. |
