The CDC recommends that everyone 6 months of age and older should get a flu vaccine every year. Remember that flu vaccine not only protects you, but it also can help protect those around you. Please help us keep our school healthy! For more information visit: www.cdc.gov/flu
Pavilions Pharmacy will be on campus to provide flu shots for SMCHS staff and students.
Please read the 2019 flu VIS and Notice of Privacy Practices before completing the 2019 Consent for Immunization Form.
On Tuesday, Sept 24th bring your completed consent with a copy of your insurance card attached.
If you are not using insurance, the cash cost is $38.00 cash or check. The following are some insurances that cover 100% of the cost of the flu shot:
• Anthem Blue Cross
• Tricare
• United Health Care
• Medicare Part B/D plan
• Aetna
• Cigna
• Federal Employee Plan
